Procedure - Request for Change of Name

Schedule of Availability of Service:

Monday to Friday 8:00 a.m. to 4:00 p.m.

Payment is until 4:00 p.m. only with no noon break

Who May Avail of the Services:

Any registered consumers of the Water District with active water service connection.

Procedure:

1. Submit required documents to the Customer Service Clerk and request for Change of Name.

2. Fill up the Change of Name (CN) Form, have it notarized and return it to the Customer Service Clerk.

3. Pay the CN Fee to any Teller from counters 1 to 5.

4. Present the Official Receipt to the Customer Service Clerk.

6. Get the original copy of the notarized CN form and the official receipt from the Customer Service Clerk.
