Procedure On Request For Laboratory Services

Schedule of Availability of Service:

Monday to Friday 8:00 a.m. to 4:00 p.m.

Payment is until 4:00 p.m. only with no noon break

Who May Avail of the Services:

Any resident/business establishment in Negros Occidental.

Requirements:

Water sample (see procedure for collection, handling and storage of water samples.)

Procedure:

1. Request Laboratory Test Forms from the Laboratory.
2. Fill up the forms and indicate the desired test parameters.
3. Get the Statement of Account from the Laboratory Technician.
4. Present the Statement of Account to any teller and pay the amount.
5. Present the official receipt to the Laboratory Technician.
6. Submit water samples (taken according to instruction).
7. Get results from the Laboratory after one (1) week.

How to collect, handle and store water samples

Standard Operating Procedures for Collection, Handling and Storage of Water Sample for Bacteriological Test

1. Clean the tap. Remove from the tap any attachments that may cause splashing and, using a clean cloth, wipe the outlet in order to remove any dirt.
2. Open the tap. Turn on the tap at maximum flow rate and let the water flow for 1 – 2 minutes.
3. Disinfect the tap. Disinfect the tap with cotton dipped in bleaching solutions (e.g. Zonrox Bleach)
No samples shall be taken from leaking taps. 4. Open the tap prior to sampling. Carefully turn on the tap and allow the water to flow for 1 – 2 minutes at a medium rate flow.
5. Fill the sterilized bottle. Remove the cap of the sterilized bottle (be careful not to touch the mouth of the bottle with your hand) and immediately fill it with water. A small air space should be left to facilitate shaking at the time of inoculation prior to analysis.
6. Label the sample. Label the sample properly with the following information:
     a.Date and time of sampling
     b.Source of Sample
     c.Sampled by: (name of collector)
7.Submit the sample within six hours after sampling.

Standard Operating Procedures for Collection of Water Sample for Chemical and Physical Analysis

1. Clean the tap. Remove from the tap any attachments that may cause splashing and, using a clean cloth, wipe the outlet in order to remove any dirt.
2. Open the tap. Turn on the tap at maximum flow rate and let the water flow for 1 – 2 minutes.
3. Fill the bottle. Fill a clean 1 liter plastic bottle with water.
4. Label the sample. Sample containers must be properly labeled. A gummed label, or a cardboard or tag securely affixed to the container should be provided with the following information.
     a.Date and time of sampling
     b.Source of Sample
     c.Sampled by: (name of collector)
5. Submit the water sample within two hours after sampling.
